Our community faces a problem

and we have some solutions.

Many unsheltered and newly housed individuals face significant barriers to achieving stability and wellness, including limited access to supportive resources, feelings of isolation, and the stigma associated with mental health and substance use challenges. Without tailored, peer-led interventions, these individuals often struggle to build trust, make positive life changes, and connect with essential services, perpetuating cycles of homelessness and instability.

AWS SMART GOALS

  • Objective 1.1

    Conduct outreach activities at least twice weekly to engage unsheltered individuals in encampments and newly housed individuals in permanent supportive housing.

    Objective 1.2

    Increase participation in peer-led group sessions by 25% within the next 12 months by implementing targeted outreach campaigns and referral partnerships.

  • Objective 2.1

    Train and certify 10 peer support specialists in trauma-informed care and facilitation skills.

    Objective 2.2

    Develop and implement a structured curriculum for peer-led group sessions that includes topics such as decision-making, wellness, and community building.

  • Objective 3.1

    Partner with at least 5 local service providers (e.g., mental health clinics, substance use treatment centers, and employment agencies) to establish a comprehensive resource network.

    Objective 3.2

    Facilitate connections to essential services for at least 100 participants annually through resource fairs, group referrals, and one-on-one support.

  • Objective 4.1

    Implement a tracking system to monitor participant progress in areas such as housing stability, substance use reduction, and mental health improvements.

    Objective 4.2

    Publish an annual impact report that details key program outcomes, participant stories, and areas for improvement.

  • Objective 5.1

    Secure $250,000 in funding through grants, donations, and sponsorships to expand services and support operations.

    Objective 5.2

    Recruit 3 additional staff or volunteers with expertise in peer support, mental health, or nonprofit management to enhance organizational capacity.
